SINUSOPATIA MAXILAR ODONTOGÊNICA EM PACIENTE INTERNADO EM CENTRO DE TERAPIA INTENSIVA - RELATO DE CASO

A sinusite maxilar odontogênica é caracterizada por um processo inflamatório dos seios maxilares decorrente de infecções odontogênicas associadas a patologias dentárias ou de intervenções odontológicas. A maioria dos casos ocorre por iatrogenias como fístulas oroantrais devido complicações de extrações dentárias, procedimentos endodônticos e falhas em implantes dentários. Grande parte dos pacientes é assintomático, sendo o diagnóstico realizado por meio de achados tomográficos. Este estudo relata o caso de um paciente, 68 anos, sexo masculino, diagnosticado com sinusite maxilar odontogênica durante o período em que esteve internado em uma Unidade de Terapia Intensiva de um hospital universitário por meio de um achado tomográfico que identificou material odontogênico associado a secreção no interior do seio maxilar. O tratamento envolveu antibioticoterapia, irrigação nasal e abordagem cirúrgica. A cirurgia envolveu a remoção de uma janela óssea para acesso ao seio maxilar, na qual foram retirados os fragmentos e realizada a curetagem. A tomografia de acompanhamento evidenciou melhora do velamento do seio maxilar. Reconhecer e tratar adequadamente a sinusite maxilar odontogênica é crucial para evitar complicações, como a disseminação da infecção, e contribuir para a melhora do prognóstico geral do paciente.
